Chronic Disease CDM and EPC plans

People with chronic health or mental health conditions may be eligible for a Chronic Disease Management CDM plan, also known as EPC or Enhanced Primary Care plans. These plans provide 5 Medicare rebated sessions (total rebate per session is $53.80 – with patients responsible for the gap fees) to be used with allied health providers per calendar year (regardless of the date of the referral). Speak with your doctor or psychologist to find out if you are eligible, and if so, to determine the best allocation of your 5 rebated sessions.

It is usual for sessions to be allocated to one or more allied health providers depending on your health care needs. For example, if you are being treated for obesity and weight loss, your plan may include 3 sessions with an exercise physiologist and 2 sessions with a dietician. If you are being treated for diabetes you may receive 1 session allocated to a diabetes educator and 4 sessions to an exercise physiologist. You can request your preferred allied health provider, and it is possible to have all 5 sessions allocated to your exercise physiologist; speak with your doctor or psychologist regarding this.

Can I book private sessions with my exercise physiologist?

Of course! Whilst medicare rebated sessions are limited, there is no limit to how many private sessions you can complete with your exercise physiologist. Most health funds provide rebates for exercise physiology services – these rebates range from $27 – $42 per session depending on your fund. Please contact your health fund to obtain information on your available rebate.

If you would like multiple clinical sessions or weekly training, a discount is available for the bulk purchase of 10 clinical sessions, these pre-purchased sessions remain valid for 12 months from the date of purchase.

Diabetes Type 2 Management

Patients with Diabetes type 2 can be referred under a Diabetes Management Plan for an initial assessment and 8 rebated group exercise physiology sessions per calendar year. If you believe you are eligible for a Diabetes Management Plan, please discuss this with your GP.

How does a Diabetes Management Plan work?

 

 

Referrals to exercise physiologists under Diabetes Management Plans are provided by GP’s under Medicare item codes 81110 (initial assessment) & 81115 (group sessions).

Once you have your referral your doctor will either give you the referral paperwork or fax your referral directly to us.

You can contact us immediately to book in for your assessment, or if we receive your paper work directly from your GP we will usually make contact with you within 5 business days.

The initial assessment attracts a $69.00 rebate and each group session attracts a $17.20 rebate. Out of pocket costs (gap fees) are $26.00 for the initial assessment and $12.80 for each group session. The full fee is payable at the time of consultation and rebates are processed into your medicare nominated account.

 

 

What is involved?

 

 

The initial diabetes assessment is used to determine your suitability for diabetes group services and will take 45 minutes.

The 60 minutes group sessions consist of both cardiovascular and strength exercise delivered by our clinical exercise physiologist.
